This compendium brings together much of the evidence about chronic disease
from the UK and around the world. The compendium was published in May
2004 by the Department of Health.
Clinical
practice guidelines for physical therapy in patients with chronic obstructive
pulmonary disease
These guidelines were produced by The Royal Dutch Physiotherapy Association
in 2003 and are concerned with the diagnostic and therapeutic processes
involved in providing physical therapy for patients with chronic obstructive
pulmonary disease (COPD).

advice lines for people with long term conditions : guidance for nursing
practitioners
This guidance document from the Royal College of Nursing (RCN) offers
advice to practitioners who provide or are developing telephone advice
services for people with long term health conditions.
Improving
care for people with long-term conditions : a review of UK and international
frameworks
'Improving care for people with long-term conditions' is a report developed
as part of a commission to identify ways to facilitate the shift of care
from hospital to community based services. Published by the NHS Institute
for Innovation and Improvement, the report was compiled on generic care
models which focus on the management of long-term conditions.
